A Team Approach to Diabetes Care

The program is staffed by certified diabetes educators and registered dietitians who work with a team of health care professionals including endocrinologists, ophthalmologists, cardiologists, nephrologists, podiatrists, primary care physicians and social workers. This team approach encourages communication and enhances the learning experience.

The Center for Nutrition and Diabetes Management follows the guidelines of Staged Diabetes Management, a comprehensive, scientifically based model for improving diabetes care. The program also features detailed follow-up with participants to assure they have met their individual goals for diabetes control.
